Stop Work Program Stop Work Authority Company Harlan McPhee North McPhee South Powerhawke Ravex Richards Company Using the Form Job Name * Job Name and/or Number Date * Stop Work Authority Every Phalcon employee or subcontractor has the right and obligation to question, stop, and/or correct, any act, task or condition you believe is unsafe. Of course, the preferred process is to always ensure that you are planning your work ahead of time, making sure that you have the training, tools and manpower necessary to achieve the outcome of work done safely. Use the following procedure to STOP WORK to correct a hazardous condition(s): 1. STOP: When a worker identifies a perceived unsafe condition, behavior, or action that may raise a concern of a danger to a person(s), equipment, or the environment, a “STOP WORK” intervention should be immediately initiated to “stop the work” with the person(s) potentially at risk. Should the SWA be utilized on site, it is expected that all employees involved will accept the SWA in a constructive manner and respond with a positive attitude. 2. NOTIFY: Notify affected personnel and supervision of the STOP WORK action. If necessary, stop all work activities that are associated with the work area in question. Make the area(s) as safe as possible by removing personnel and stabilizing the situation. 3. INVESTIGATE: Affected personnel will review and discuss the situation and come to an agreement on the STOP WORK action. If all parties agree that the condition or task is safe to proceed without additional modifications, (e.g., the STOP WORK initiator was unaware of specific information or circumstances), the SWA is complete at this point and no further steps are needed. When opinions differ regarding the validity of the stop work utilization, the investigation group should be expanded and elevated to management who may bring subject matter expert. If it is determined and agreed that the SWA is valid, a documented process using the STOP WORK Intervention Form should be used (located in the Safety Manual) to ensure all affected and authorized parties are aware and part of the corrective action. 4. CORRECT: The condition or action that posed a threat or imminent danger to an employee, equipment, or the environment must be corrected before restarting work. Modifications and repairs to the affected area, tools or equipment including potential change in process will be made as needed before resuming work. 5. RESUME: When the reported condition has been inspected by supervision and all safety issues have been appropriately resolved, the area will be reopened for work. 6. MONITOR: Measuring and evaluating the level of control that is achieved is an essential step in ensuring that the issue(s) is entirely abated. While it is the desired outcome of the STOP WORK action to ensure the identified safety concerns are addressed to the satisfaction of all involved persons before the resumption of work, it is prudent to ensure the corrective action(s) taken was appropriate and follow-up inspections conducted. Further investigation and corrective actions may be required to identify and address additional root causes discovered from such monitoring activity. 7. COMMUNICATE: An authorized person should publish the STOP WORK action details to the safety department, supervisory staff, managers, and employees outlining the issue, corrective action(s) taken, and lessons learned. Senior management will review all stop work reports and may identify additional follow-up to be done. SWA is a lot like PPE in the hierarchy of controls of workplace hazards - it is the last line of defense. You should always strive to provide a safe workplace and safe work upfront.
