At Phalcon, the 4 types of SIF injury sources that are most common are contact with electricity, struck by, slips & falls and caught in. Let’s review each of these hazards. #1 - Contact with Electricity: Linemen and electricians suffer the highest number of fatalities related to contact with electricity in the construction industry and are exposed to that hazard often. However, linemen and electricians are also highly trained in mitigating electrical contact hazards, they are equipped with the proper PPE and tools and are therefore expected to accomplish electrical work safely, with no incidents or injuries. Hazard Mitigation and Prevention: • Use of LOTO – each circuit that could become energized must be locked out, by each employee that will be working on it, at its source, followed by verifying the absence of voltage. • Test before you touch – Always test for voltage before working on a circuit, even if you know you locked it out, and especially if you stepped away for break or lunch. o We had a near miss earlier this year when another contractor cut a lock off that we installed and turned the circuit on. The hazard was identified just before there was an injury, a catastrophic outcome was prevented on that day. • 20/20/20 – be aware of your surroundings. o We have experienced several shock injuries due to exposure to energized wires adjacent to and unrelated to our work areas. #2 - Struck By: Struck-by-object incidents happen when a worker is hit by any object or piece of equipment that is moving, falling, swinging, or rolling. This can include tools, machinery, vehicles, or falling items like tools or debris. Hazard Mitigation and Prevention: • 20/20/20 – be aware of your surroundings. Keep yourself out of pinch points. It’s good practice to think about the worst-case scenario and then put yourself out of harm’s way in case that scenario becomes a reality. • Pay attention to your hands’ position when using power tools. • Never work under a load or adjacent to an unstable load. • Be familiar with equipment manuals, safety guidelines, and warning labels. Regular equipment inspections and maintenance should be conducted to promptly identify and address any defects or malfunctions. Always follow the manufacturer’s recommendations. #3 - Slips and Falls Most of the fall from elevation exposures we see come from falls from ladders. Employees working on a ladder can suffer an injury from either loss of balance, miss step when coming down or the ladder slips out from under them resulting in a fall. We have also seen falls on the same level that resulted in the employee striking their head on the ground. Hazard Mitigation and Prevention: • Evaluate alternatives to ladder use, i.e. scissor lifts, articulating boom lifts or scaffolding that will be best suited for the task. • Inspect the ladder before use. Remove it from service if there are cracks in the fiberglass, bent steps, missing hardware or worn feet. Failure of any of the components can lead to a fall. • Set up the ladder correctly and in the proper location so the work area can be accessed without over-reaching or having to stand on the top two steps. #4 - Caught In Caught in events result in crushing type injuries, resulting from a person or body part being squeezed, caught, crushed, pinched, or compressed between two or more objects or between parts of an object. Hazard Mitigation and Prevention: • Proper Positioning - Avoid being positioned between moving objects and stationary objects like other vehicles, walls or stacked materials. • Know your equipment - Understand where pinch points, shear points, and pull-in areas are on tools, equipment and machinery you operate. • Avoid loose clothing, jewelry and loose long hair around rotating equipment. Although gloves may be tight fitting, they can pose a risk of injury around rotating equipment, pay attention to your hands’ positioning. o We have had numerous instances of them being caught up in rotating drill chucks causing injuries. • Communication - Maintain clear communication with other workers, especially when operating heavy machinery.
