Lockout / Tagout Procedures Lockout / Tagout Procedures Company Harlan McPhee North McPhee South Powerhawke Ravex Richards Company Using the Form Job Name * Job Name and/or Number Date * Lockout / Tagout Procedures Lockout/Tagout Procedures Before any maintenance, construction, demolition, tie-in, inspection or servicing of equipment (electrical, mechanical, steam or other) that requires entrance into or close contact with machinery, equipment or power sources, the power shall be disconnected and locked out. Electrical It is the expectation of Phalcon, that if you are going to perform work on a piece of equipment that could become energized, that YOU lock it out, so that you have control of the energy source and that no one else can turn it on and accidentally injure you. That means that you put your unique lock on the isolation point/s (or lock box), verify the absence of voltage with a properly rated and functioning voltage tester. Do not trust anyone. It’s your life you are potentially putting on the line, you lock it out. Don’t rely on the foreman or your best friend. When verifying the absence of voltage, ensure that you test the device on a known power source, before and after testing for the absence of voltage, to ensure that the detector is functioning properly. Also note that until you have locked out the source and verified the absence of voltage, the equipment must be considered energized and all appropriate PPE, including FR clothing and rubber electrical protective gloves must be worn. Other Energy Sources There are many potential sources of energy that we can run into such as thermal (too hot or cold), hydraulic, chemical, mechanical or potential energy (energy associated with an object because of its position, i.e. elevated). Each one of these energy sources must be locked out to help ensure that we can complete the work safely. Locks and Tags Removal Locks and tags will be attached and removed only by the individual employee who put it on. There are procedures to follow if removing another employee’s lock is necessary, but in general, NEVER remove another employee’s lock. Contact your supervisor if you need locks, tags or device to comply with our LOTO procedures. In closing, we work around hazardous energies often. Don’t take them for granted. One attempted shortcut to try to save some time or effort, could be the last decision you make. It is not worth it! Make sure you follow the LOTO procedure and you will have a long and safe career.
