We have experienced over 90 injuries in the last 5 years. Below are some of those incidents. Like any other Lessons Learned reports that circulate, the intent here is to not place blame or fault with an individual, but more to make everyone aware of what happened so we can learn from them and when the situation arises again, the work can be completed without injuries. All of the injuries listed here were OSHA record-able injuries. Pinch Points While loading wire into a wire cart with other employees. The employees finger got caught in the pocket where the post is seated that the wire rolls go onto. His middle finger on his right hand was smashed into the edges and he suffered a laceration as a result that needed stitches. While unloading pipe off truck, worker's middle finger on left hand got caught between pipe. Employee was moving a 400 amp disconnect. As he was setting it down, he lost control of it and dropped it on his finger, he was wearing gloves. The employee had to take skin from the palm of his hand to re-build the tip of his finger and splinted the finger for the broken bone. Employee was moving 300 lb transformer off a housekeeping pad and dropped it on his finger. Employee was rolling a rope on a wood spool and his finger got pinched between the spool and a railing. Recognize the hazards and think worst case. Carefully planning your work, having a good daily tailboard to discuss the work and things to look out for will help prevent these injuries. Twisting & Awkward Positions An employee was unloading a bundle of 1" EMT pipe from a service truck and stepped back and twisted at the same time causing a strain of muscle and tendon in lower back. An employee was moving material and tools down a stairwell and twisted his knee. The employee had a bone fragment in his knee and a torn meniscus Employee was loading strut from a forklift to a truck and twisted his knee An employee was carrying a pole light and twisted straining his lower back An employee was carrying one sheet of plywood to load into a truck at an awkward position. Employee was stocking wire in a lock box. He bent over with a heavy load and pulled something in his back. Your body can only handle a certain amount of stress. When you are carrying extra weight and twist, this will often exceed the limits resulting in an injury. Follow basic lifting best practices and avoiding twisting will help prevent these types of injuries. Another couple of sources of injury are pulling cable and carrying or moving materials. The repetitive motion and force exerted while manually pulling cable and wire causes many strains and sprains. For short wire pulls with smaller wires, manually pulling can be accomplished safely. For larger wires/cable with longer runs and bends, use of mechanical pullers will reduce the risk of injury. Many injuries are experienced manually moving materials around the project. A few things that can be done to help this are to try to have materials delivered close to where they will be used, use carts and other wheeled equipment instead of carrying things and get help lifting heavy or awkward items. Remember the 50 lb guideline. Plan your lift and work smart. Think about these and how they could apply to the work that you are doing. Learn from these incidents so you don’t suffer the same pain as those who have already been injured.
